I have read this Consent Form and, under Law No. 6698 on the Protection of Personal Data,
I accept, declare, undertake and give prior consent to Prof. Dr. Süleyman TAŞ collecting, recording and processing
my personal data lawfully.
In accordance with Law No. 6698 on the Protection of Personal Data and the Regulation on the Protection of Personal Data,
I accept, declare and undertake the statement titled Processing of Personal Health Data and Ensuring Its Confidentiality.
I undertake the following:
I consent to Prof. Dr. Süleyman TAŞ processing my personal data, including my health data and any personal data that identifies or defines my sensitive personal identity, where sharing with other persons or organisations verbally, in writing and/or electronically is required under the relevant legislation.
My sensitive personal data and health data described below may, within the scope defined in the Law,
be processed under Law No. 6698 on the Protection of Personal Data, the Regulation on the Processing of Personal Data, and the rules on Health Data and Ensuring Privacy.
• Data clearly belonging to an identified or identifiable natural person, partly or wholly processed, automatically or non-automatically as part of a data recording system:
Identity information such as driving licence, identity card, passport, tax number, health insurance number, signature, vehicle plate, name and surname, ID/passport number, nationality, mother's and father's names, place of birth, date of birth and sex.
• Contact data clearly belonging to an identified or identifiable natural person, partly or wholly processed, automatically or non-automatically as part of a data recording system:
Telephone number, address, e-mail address, fax number, IP address.
• Physical premises data clearly belonging to an identified or identifiable natural person, partly or wholly processed, obtained automatically or non-automatically as part of a data recording system:
Camera and security recordings, fingerprint records, etc. taken on entry to or during a stay at the premises.
• Photographs and camera recordings clearly belonging to an identified or identifiable natural person (excluding records covered by Physical Premises Security Information), personal data in documents and copies of documents containing personal data.
• Special categories of personal data under Article 6 of Law No. 6698 on the Protection of Personal Data:
All kinds of health data, blood group information, biometric data, etc.
• Personal data clearly belonging to an identified or identifiable natural person, partly or wholly processed as part of a data recording system:
Data used in receiving and assessing all requests and complaints addressed to Prof. Dr. Süleyman TAŞ.
